Short answer
Shoulder discomfort while side sleeping is usually about height, not about the pillow brand. Lying on your side, your shoulder lifts your torso off the mattress, so the gap between mattress and head is at its largest — often larger than a low pillow can fill. Why side sleeping is the hardest case for a fixed-height pillow
On your back, the gap you need to fill is small. On your side it is roughly the distance from the side of your head to the point of your shoulder, minus however far your shoulder sinks into the mattress. That is why two people can rate the same pillow completely differently: a softer mattress lets the shoulder sink and reduces the height needed, and a firm mattress increases it. A practical way to check your own height need
- Lie on your side on your own mattress with your current pillow, in your normal position.
- Have someone look from the front: is your nose in line with the centre of your chest, or is your head tipped up or down?
- If your head tips down toward the mattress, you need more height than you have.
- Measure the pillow you are using now at its thickest compressed point — that is your working reference for any new pillow.

Pressure on the shoulder itself
A pillow supports your head and neck; it does not unload your shoulder. If the pain is in the shoulder joint rather than the neck, mattress comfort layers, arm position and the underlying cause matter far more than pillow choice. A pillow that is too low can make things worse by letting your head drop and your upper body rotate, but a correctly-sized pillow does not treat shoulder pathology.

- Shoulder pain that wakes you repeatedly, or that has lasted several weeks.
- Loss of movement, weakness, or pain following a fall or injury.
- Numbness or tingling down the arm or into the hand.
